Diabetes Management Plus for Fully Insured and OptiFlex in 2025
Beginning January 1, 2025, as fully insured and OptiFlex groups renew, Premera Blue Cross will offer members Diabetes Management Plus from Teladoc. Diabetes Management Plus is a simple and convenient way for members to manage their diabetes at no out-of-pocket cost to the member.
Diabetes Management Plus includes:
- A connected glucose monitor
- Personalized tips with each blood glucose check in
- Real-time support when members are out of range
- Strip and lancet reorder from the meter or app
- Optional family alerts to keep everyone in the loop
- Ability to send a health summary report to their provider directly from their meter or app
- 24/7 personalized coaching by phone or text
Why Diabetes Management Plus is different
Many living with diabetes can often be living with another chronic condition. Many adults with diabetes also have hypertension, are overweight or obese, and are two to three times more likely to struggle with depression.1,2
Along with tools to help members manage their diabetes, members can also access other support programs, including Hypertension Plus, Standard Weight Management, and Digital Mental Health.
The combination of these programs offers Premera members a holistic approach to managing their whole health and not just their chronic conditions.
