HMO 101: Helping Members Manage Their Chronic Conditions

Next month, Premera Blue Cross HMO (PBC HMO) members will receive a communication from us that talks about chronic care management. As an HMO 101 communication, this is aimed at improving healthcare literacy around our health plans and also healthcare in general.

We offer five recommendations on how they can stay on top of their care to feel better and reduce pain. It also includes a helpful reminder that through their health plan, they have access to:

  • Personal Health Support
    • Members can work with case managers to address health concerns.
  • Health risk assessment
    • Members can securely share information about their health concerns and a case manager will reach out to them, with their consent.
  • Teladoc Diabetes Management
    • Members with prediabetes, diabetes, and hypertension can track their condition and get better insights.

Personal Health Support and our health risk assessment can help undiagnosed members address health concerns that they may have overlooked. These are resources that are available to all PBC HMO members through Enhanced Case Management, and they can help reduce the total cost of care for both employees and employers.
